Steps to Take Prior to Your Dental Visit
It can be very helpful if you bring information with you that helps shed light on your dental history. If you have received care at another office, please contact them and request to have your radiographs sent to our office. We can coordinate with other offices directly, but we need your permission first.
Some offices require a signed request form in order to send us your dental records. You can stop by our office to pick up a copy or download and fill out our Radiograph Request Form to save yourself the trip.
New Patient Paperwork:
There are a couple of forms that you will need to fill out during your first visit. To help save time when you come to the office, please download the New Patient Forms and fill them out ahead of time. If you provide them to the front desk when you arrive, it will save valuable time and allow us more opportunity to spend with you.
If you are unable to fill out the forms ahead of time, please arrive 10-15 minutes prior to the beginning of your dental appointment to complete the necessary paperwork. We want to make sure there is plenty of time to perform a thorough exam and answer any questions you may have.
Dental Insurance and Membership Plan
At bohn dental, we strive to provide care that is accessible to everyone. We accept most dental insurances and will help coordinate your billing. In addition, we offer financing and payment plans if needed. For those without insurance, we are proud to offer our very own membership plan at bohn dental.
