Figuring out what is important to you is critical moving forward. Your first visit provides the opportunity to sit down and get to know each other. At bohn dental, your care is centered around you. We set aside extra time that allows us to figure out exactly what your goals are and what you want to accomplish.

Steps to Take Prior to Your Visit:
It can be very helpful if you bring information with you that helps shed light on your dental history. If you have received care at another office, please contact them and request to have your radiographs sent to our office. We can coordinate with other offices directly, but we need your permission first.
New Patient Paperwork:
There are a couple of forms that you will need to fill out during your first visit. To help save time when you come to the office, please download the new patient forms and fill them out ahead of time. If you provide them to the front desk when you arrive, it will save valuable time and allow us more opportunity to spend with you.
If you are unable to fill out the forms ahead of time, please arrive 10-15 minutes prior to the beginning of your appointment to complete the necessary paperwork. We want to make sure there is plenty of time to perform a thorough exam and answer any questions you may have.
